  • This form is to be used to request payment for individuals performing a service for which an invoice will not be provided.

IRS W-9 Form

  • This form is required of any new vendor providing a service or when requesting payment of a honorarium.  An updated W-9 form should be submitted for a vendor with any change in tax information/address.  A W-9 form ensures the collection of accurate/updated information that is signed/verified by the vendor.  Also, as yearly audit regulations become more extensive having this information will be pertinent to keep us in compliance with all accounting and IRS regulations. For further information please see our Roanoke College - W-9 Form and 1099 Tax Reporting Guidelines (this document is also accessible in the Policies section of the Accounts Payable and Purchasing Page).

Maroon Card Financial Aid Transfer

  • If you anticipate a credit balance on your account after all of your finanical aid has been applied, you are eligible to request a transfer of funds to your Maroon Card via this form.

Missing Receipt Disclosure

  • Per policy it is the Roanoke College VISA Cardholder's responsibility to obtain itemized transaction receipts from the vendor each time the credit card is used.  Cardholders must attempt to get the receipt from the vendor.  If the receipt is not available, the cardholder must attach a completed Missing Receipt Disclosure for each receipt missing.

Petty Cash Request

  • This form is used for receiving reimbursement for amounts under $50.00.  Please submit this form in person to the Cashier in the Business Office.

Purchase Order Requisition

  • This requisition should be submitted for goods where the total purchase is $5,000 or more.  Requisitions must be submitted and approved before the goods are ordered. Please allow two business days after receipt in our office for the requisition to be processed and approved.  For full details please refer to the Roanoke College Purchasing policy.

Transfer to Maroon Card from Credit on Student Account

  • Excess credit funds on your student account may be transferred to the Maroon Card with permission from you and/or your parent instead of, or in addition to, receiving a Roanoke College issued refund check.

Travel - Expense Report

Travel Authorization Form

  • Effective 7/1/2014 and until further notice, this form must be completed for all staff travel/trips for conferences and professional development prior to commitment of College funds (travel for recruitment and fund raising do not need prior approval by the President).  Allow at least two weeks for processing, prior to need for commitment of funds.  This form is in addition to the Travel Advance Request and/or Travel Expense Report.

On-Campus Room Change Request

  • This form is used to submit a room change request to change housing on-campus.

Request for Commuter Student Status

  • A student can request to be a commuter student if he/she meets one of the following criteria: 

    • He/she plans to reside within 30 miles of campus with a parent/guardian, grandparent or sibling
    • He/she is married
    • He/she is at least 23 years old
    • He/she is a military veteran
    • He/she is or will be enrolled part-time during the term seeking off campus housing.
    • He/she is a 5th year senior

    Deadline to apply for commuter status is June 15th for the upcoming academic year for returning students. 

Request for Waiver of Residency Requirement for Medical/Psychological Reasons

  • Appeals are due February 14th by noon in order to be considered for the fall 2018. Late appeals and incomplete appeals will not be considered. Students may appeal their residency requirement two times during the academic year (once in the fall and once in the spring) for financial and/or medical reasons only.  Financial appeals will need to be submitted to Residence Life & Housing.

    Submission of this form to Roanoke College does not guarantee off-campus release. Students are required to participate in housing selection unless approval is granted.

Summer Housing for Student Employees

  • The College may provide summer housing on a limited basis for student employees.  Please note that the value of “qualified campus lodging” provided free of charge is considered by the IRS to be a taxable benefit, and taxes on the value of the housing will be withheld from each payroll paid during your occupancy.  The taxable value will also be reported on your year-end W-2.  With the exception of jobs which require the student to live in a residence hall (Resident Advisors and certain Summer Conference employees) all other positions will require that the College calculate the value of free housing at a rate of $25.00 per week day for the period May 13 to July 29 based on actual occupancy.

    Student Employees: This completed form must be signed by the supervisor (or department head if different than the supervisor) and by the divisional VP and returned to the Residence Life and Housing office, no later than April 16, 2018.

    Criteria for Residency: 

    1. Student must be enrolled and deposited for the subsequent fall term.
    2. Student must live beyond the Roanoke Valley Area (30-miles from campus.)
    3. Student must be performing a “mission critical” service to the institution as determined by the respective divisional Vice President.
    4. Student must be employed by the sponsoring department a minimum of 20 hours per week for the period in which the student occupies summer housing.

    RC Student Health & Counseling Services utilizes a triage screening system for initial counseling appointments.  The purpose of triage screening is to assess your current concerns/challenges and determine the most appropriate treatment options for addressing your clinical needs. The screening consists of questions, including demographics (i.e., age, year in school, address), presenting problem, treatment history, substance usage and other concerns.
